Unite Against Sepsis: Collaborating to Reduce Sepsis Mortality and Improve Care Coordination

Event Description:

Sepsis is the leading cause of death in U.S. hospitals. Sepsis is also the leading cause of readmissions to hospitals, with as many as 19% of people originally hospitalized with sepsis re-hospitalized within 30 days.

Join this collaborative learning series aimed at supporting your facility in reducing sepsis mortality and improving care coordination. This learning collaborative has been developed using the newly released Centers for Disease Control and Prevention (CDC) Hospital Sepsis Program Core Elements. This series will include live presentations from subject matter experts, case study discussions and guidance on using the Institute for Healthcare Improvement’s (IHI) Breakthrough Series model to improve your sepsis program.

  1. January 24, 2024 – Sepsis Core Elements: Leadership, Accountability and Multi-Professional Experience
  2. February 21, 2024 – Sepsis Core Element: Action
  3. March 20, 2024 – Sepsis Core Element: Tracking and Reporting
  4. April 17, 2024 – Sepsis Core Element: Education (and Patient and Family Engagement)

Objectives:

Objectives of this event include:

  • Develop your understanding of the CDC’s Hospital Sepsis Program Core Elements through interactive discussion with subject matter experts and facilitated peer-to-peer discussion.
  • Assess your facility’s current sepsis program, identify opportunities for improvement, draft goals and practice using the Plan-Do-Study-Act (PDSA) cycle to implement evidence-based interventions.
  • Collaborate with multi-disciplinary teams and across care settings to identify gaps in post-discharge sepsis care coordination and brainstorm creative solutions.
  • Learn strategies for analyzing your facility’s sepsis data to routinely assess progress toward your facility’s sepsis care goals.
