Patients & Families

We Are Better Together!

Together, we can:

  • Ensure every person has an opportunity to achieve optimal health regardless of race, ethnicity, gender, disability, community, geographic location, social economic status or health literacy
  • Create a healthcare system that puts patients first by providing Person and Family Centered Care
  • Reduce opioid-related harms
  • Increase access to behavioral health services
  • Prevent healthcare-related infections
  • Prevent and manage diabetes, heart disease and chronic kidney disease
  • Avoid hospital readmissions and unnecessary emergency room visits
  • Improve the quality of nursing home care

Telligen is working to make the healthcare in our four-state region the best in the nation. But we can’t do it alone. We need your help! And, we are here to help you too! View our recruitment flyer here for more information.

We actively seek to partner with patients, Medicare beneficiaries, family members, and caregivers who want to become more engaged in their care and improve their overall healthcare experience through access to education and resources. Your voice is your most important tool to understanding your health and getting the care you need. We want to help you be more confident in speaking up about what matters most to you, making healthcare decisions, participating more fully in your own care and sharing your feedback at the systems and policy level.

Get Involved

We believe “patients and families are partners in defining, designing, participating in and assessing the care practices and systems that serve them to assure they are respectful of and responsive to individual patient preferences, needs and values. This collaborative engagement allows patient values to guide all clinical decisions and drives genuine transformation in attitudes, behavior and practice” (Centers for Medicare & Medicaid Services Person & Family Engagement Strategy).

To learn more about ways you can partner with Telligen and share the patient or caregiver perspective, please complete our interest form below. You can decide your level of involvement so that it fits with your schedule. We will follow up with you to learn more about your interests, answer any questions and discuss upcoming opportunities.
