Sepsis is the leading cause of death in U.S. hospitals. Sepsis is also the leading cause of readmissions to hospitals, with as many as 19% of people originally hospitalized with sepsis re-hospitalized within 30 days.
Join this collaborative learning series aimed at supporting your facility in reducing sepsis mortality and improving care coordination. This learning collaborative has been developed using the newly released Centers for Disease Control and Prevention (CDC) Hospital Sepsis Program Core Elements. This series will include live presentations from subject matter experts, case study discussions and guidance on using the Institute for Healthcare Improvement’s (IHI) Breakthrough Series model to improve your sepsis program.
